- Treatment summaries and vet referral details upon request
These documents are commonly required by pet insurance providers to support claims for hydrotherapy and physiotherapy services.
What You Need To Do:

- Submit your claim directly: You’ll need to send the completed forms and supporting documents to your insurance company.
- Help us help you: Fill out our Insurance Information Form so we can respond quickly to any questions or requests from your insurer.
